What can US Birth facilities do now?

Birth facilities can make a commitment to improve breastfeeding policy, training and practices. In turn, they can receive a Certificate of Intent for their commitment to creating an environment supportive of the Ten Steps to Successful Breastfeeding.

The vast majority of hospitals in the US do not fulfill the Ten Steps, but hospitals in almost every state are eager to work toward that end. Among these institutions are both large and small hospitals, for profit and not-for-profit hospitals, teaching hospitals, and hospitals at various stages of development in their breastfeeding education and support services, as well as birthing centers.

The Certificates of Intent are given out on an honor system. The self-appraisal process does not verify compliance with all of the Ten Steps, but is rather a beginning point. Receiving the Certificate of Intent is not equivalent to receiving the Baby-Friendly designation, but rather recognizes those US hospitals and birthing centers that are working toward applying the Ten Steps.

The Baby-Friendly Award process requires an on-site survey which is conducted after the hospital or birthing center indicates its readiness for assessment. During the intensive two or three day visit, many staff, physicans, and mothers are interviewed. Only after the facility has passed this assessment does it receive the designation of being a Baby-Friendly Hospital.

How can Hospitals and Birthing Centers apply for a Certificate of Intent?

Obtain an introductory packet and self-appraisal tool.

Assemble a team to review and complete the self-appraisal tool.

Seek administrative support

Submit a Certificate of Intent packet containing the following items:

1 - A completed copy of The Self Appraisal Tool (pdf) . This questionnaire is designed as a self evaluation and education tool that assists interested staff in identifying strategies for optimal care for breastfeeding mothers and babies
2 - A Letter of Intent signed by the CEO of the hospital or other appropriate hospital administrator that includes the following items:
brief description of activities and any existing supporting documentation (e.g., a copy of policy) to demonstrate which of the Ten Steps the institution has already accomplished;
brief description of actions planned (e.g., convene in-house task force, implement policy, provide training) and problems foreseen in accomplishing the Ten Steps
general timetable for breastfeeding promotion and support activities to accomplish the Ten Steps
any other information you would like to include that could be shared
3 - An Annual Fee to cover the costs of participation in the initiative. Fees are based on number of births annually and currently range from $550 to $900.
Also, the following should be submitted with the letter of intent or within six months after receiving the Certificate of Intent:
a copy of the hospital’s written breastfeeding policy
documentation of breastfeeding training currently given to hospital personnel or a plan for establishing a training program
documentation of the breastfeeding rates at the hospital and the methodology used to collect these data or a plan to document these rates

A Certificate of Intent will be awarded after a review for completeness (not correctness). The Certificate of Intent is the first step to becoming a Baby-Friendly Hospital; it helps to identify the achievements and challenges each facility faces.

Is help available?

Yes! Baby-Friendly USA staff members are available to provide technical support in the preparation of an application for a Certificate of Intent. Assistance is also available for increasing the awareness of the Initiative in your community for individuals and organizations not affiliated with hospitals. In addition, a series of materials to help hospitals work toward implementation of the Ten Steps is also available (see order form).

A guidance document may be downloaded to assist with overcoming barriers. The report, completed with the government support, is a compilation of strategies used by the existing Baby-Friendly facilities.

Why participate in the Baby-Friendly Hospital Initiative?

Participation in this initiative provides several possible benefits for birthing facilities:
quality improvement: many of the ten steps are easily adaptable as QI projects.
cost containment: increased breastfeeding rates can have impact on many health care costs from postpartum hemorrhage, to decreased incidence of ear infection.
public relations/marketing: families who feel adequately supported during the vulnerable postpartum days can speak powerfully for a birth facility.
prestige: The receipt of this international award is an achievement to celebrate!
